Services at
Home
Hospitalization is not a prerequisite for home health care services.
  • Discuss your needs with your physician who will contact the Community Health Association.
     
  • If you are hospitalized or admitted to a rehabilitation center, inform the discharge planner that you would like the services of the Community Health Association.
     
  • You and your family may contact the Community Health Association directly.

 
Payment Information
The Community Health Association is a Medicare/Medicaid certified home health agency and member of the Visiting Nurse Associations of America. The Community Health Association is certified by Medicare and Medicaid to provide home health care services to the beneficiaries of these insurances. HMO eligibility is determined at the time of referral. For Medicare reimbursement, clients must be home-bound, need skilled services on an intermittent basis and meet other eligibility requirements. Our staff will address your questions pertaining to insurance benefits and coverage.


Office hours are from 9:00 a.m. to 3:00 p.m., Monday through Friday except holidays.

Nurse availability is between 9:00 a.m. and 9:00 p.m. seven days a week.
